About Us
The M-CARE Project is a multi-country initiative focused on improving primary health care for adults living with multiple long-term chronic non-communicable diseases (NCDs) such as diabetes, hypertension, and mental health conditions. Operating in Ghana, Kenya, and Uganda, M-CARE is addressing a growing health crisis in Sub-Saharan Africa, where fragile health systems struggle to manage the rising burden of NCDs.
M-CARE works to shift healthcare from fragmented, disease-specific treatment toward an integrated, person-centered approach. The project aligns with WHO’s PEN (Package of Essential NCD Interventions) and mhGAP-IG (Mental Health Gap Action Programme) guidelines to empower frontline health workers in low-resource settings.

A future where everyone in Sub-Saharan Africa - regardless of income or location - has access to integrated, high-quality, and person-centered care for both physical and mental health conditions.
Mission
To improve the lives of people with multiple chronic NCDs by strengthening primary health care systems through innovative, evidence-based strategies tailored to local contexts in Ghana, Kenya, and Uganda.
Objectives
→ Enhance primary health care delivery for adults (18+) with cardiometabolic and mental health conditions in SSA.
→ Design and implement integrated care strategies using WHO’s PEN and mhGAP-IG guidelines.
→ Adapt interventions to fit the diverse socioeconomic environments of the four countries.
→ Leverage social media and community engagement to expand reach and ensure sustainability.
→ Evaluate outcomes such as improved engagement, retention in care, and clinical effectiveness.
→ Contribute to global health goals, including a 25% reduction in premature deaths from NCDs by 2030.
