M-CARE Project

Transforming Care for Chronic Conditions and Mental Health in Sub-Saharan Africa

M-CARE is an initiative focused on improving healthcare for people with multiple chronic conditions and mental health challenges in Sub-Saharan Africa.

We work across Ghana, Kenya, and Uganda, adapting WHO’s evidence-based guidelines to create integrated, patient-centered care at the primary health level.

Our mission is to strengthen early detection, treatment, and long-term care, helping reduce premature deaths from non-communicable diseases and building sustainable health systems across Africa.

A new model of integrated, patient-centered care

M-CARE delivers a new model of integrated, patient-centered care for managing multiple chronic and mental health conditions in Sub-Saharan Africa. By combining WHO’s PEN and mhGAP-IG guidelines with local insights, we empower primary healthcare systems to detect, treat, and manage cardiometabolic and mental health conditions effectively. Our innovative approach ensures early intervention, sustainable health improvements, and stronger communities across Ghana, Kenya, Malawi, and Uganda.
